Intervenciones de enfermería en protocolos ERAS: efectos sobre complicaciones, dolor y estancia hospitalaria

Resumen

Los protocolos de recuperación mejorada después de la cirugía (ERAS) han transformado el cuidado perioperatorio al integrar intervenciones basadas en evidencia orientadas a disminuir la respuesta al estrés quirúrgico y acelerar la recuperación funcional. La presente revisión bibliográfica analiza el papel de las intervenciones de enfermería dentro de ERAS y su efecto sobre las complicaciones posoperatorias, el dolor y la estancia hospitalaria. La evidencia reciente muestra que la enfermería constituye un eje operativo del protocolo mediante educación preoperatoria, reducción del ayuno, preparación nutricional, vigilancia de la analgesia multimodal, prevención de náuseas y vómitos, movilización temprana, inicio precoz de la vía oral, retiro oportuno de sondas y drenajes, y refuerzo de la adherencia del paciente. En conjunto, estas acciones se asocian con menor incidencia de complicaciones, mejor control del dolor, menor consumo de opioides y reducción significativa de la estancia hospitalaria, sin incremento consistente de la mortalidad ni de los reingresos. Asimismo, la adherencia al protocolo y la capacitación del equipo condicionan gran parte de los resultados clínicos. Se concluye que las intervenciones de enfermería en ERAS no solo complementan el manejo multidisciplinario, sino que representan un componente estratégico para optimizar la seguridad, la recuperación y la eficiencia hospitalaria en el paciente quirúrgico.
